Packages

Packages are designed to provide additional savings to your routine care. Packages are nonrefundable and expire 6 months from purchase date. Package value may be exchanged for assisted stretch and/or chiropractic services offered at Aviva Family Chiropractic.

Cancellation and No Show Policy

If you need to cancel your appointment, please cancel online, call/text our office (787) 417-7794 or email us within 2 hours of your scheduled appointment. A failure to cancel a scheduled appointment without a 2 hour notice or a "No Show" may be charged a cancellation fee of $25.00 to credit card stored on file.

Changes

We reserve the right, at our sole discretion, to modify or replace these Terms at any time. Please review our Terms and Conditions periodically to review any changes. What constitutes a material change will be determined at our sole discretion.
